5-Year Strategic Plan

Palm Health Foundation’s story is one of continuous evolution. We’ve grown from a traditional hospital conversion foundation in 2001 to a nationally recognized community-led funder that is building a culture of health by investing in the social, behavioral, and environmental conditions at the root of health inequity.

We know that health is more than healthcare. Our health is determined by the many experiences we have in a lifetime – from the neighborhoods we live in, to the parks our children play in, to the faith communities in which we participate. 

Rather than prioritize the systems designed to help people, our funding model prioritizes the experiences of people. Our neighbors’ lived experiences shape and inform our solutions to Palm Beach County’s greatest health needs and help us build on what’s already working to better health.

At Palm Health Foundation, we lead with dignity for those we serve, centering our grantmaking and initiatives on the human experience and listening to the voices of our neighbors. Through this collaboration, we are building a culture of health for all who call Palm Beach County home.
